Pure Health At Home provides in home medical care and personal living assistance to help patients heal from injury, recover from illness and live as independently as possible.
Our team of doctors, nurses and therapeutic specialists work with you to help you understand your condition and the steps you need to take to get better. We then provide a wide range of at home services to assist you through the process of regaining strength and balance, accommodating changes in activities of daily living, and improving cognitive and speech abilities. Most importantly, we seamlessly assess and refine our treatment plans and reconfigure our team as needed to adapt to each patient’s changing circumstances either as they recover from their injury or illness or as their condition progresses, such as in the case of chronic diseases.

Our team of fully licensed and trained physicians, nurse practitioners, nurses, and specialized therapists offer deep expertise providing compassionate, patient-centered care in the home environment. We treat the whole person, paying attention to your physical, mental and emotional condition and helping you improve in body, mind and spirit. We know it can be uncomfortable welcoming strangers into your home, but before long you’ll get to know your team members — attentive and supportive professionals who will make you as comfortable as possible while you recover your health.
Coordination of care across your healthcare team
Pure Health At Home takes responsibility for making sure every member of your care team is apprised of your condition as it changes, day by day. That includes keeping your physicians up to date on your progress, keeping all your therapists and aides on your Pure Health At Home team aware of each other’s actions, and keeping your family members informed about how you are doing.
Involving your family caregivers in our care plans
We encourage patients to include family members or personal caregivers in your care for added support during the hours Pure Health At Home can’t be with you. You decide who you want to involve and how much information you want us to share with them. Then we communicate with your family caregivers as frequently as you wish — often daily or weekly. We can also train family caregivers to provide some important forms of support, such as medications management, changing dressings on wounds or helping with supplemental breathing exercises. If you choose not to involve any family caregivers in your recovery, that’s fine, too.
