What is at home rehabilitation?
Rehabilitation therapy helps people who have had major surgery, an injury or a disease that has limited their ability to function normally. Rehabilitation focuses on the whole person. It is designed to build strength, increase mobility, and regain thinking and speaking abilities so that you can function as independently as possible and go back to your daily life. At home rehabilitation provides a full spectrum of rehabilitation services in the patient’s home instead of in a skilled nursing facility.
What services do you offer?
In addition to skilled nursing for a wide array of professional medical support, Pure Health at Home offers the three most common types of rehabilitation: physical therapy, which builds physical strength, balance and mobility through a carefully developed plan of physical movements and exercises; occupational therapy, which focuses on helping a person identify and relearn or learn new ways to do everyday activities, such as walking, bathing, grooming, eating, getting in and out of bed, or doing housework; and speech and language therapy, which assists with regaining the ability to understand language, speak, swallow and use the mouth normally. In addition, we offer specialized therapies, including pulmonary therapy to help with respiratory conditions, neurologic rehabilitation following brain or spinal cord injuries, cardiac rehabilitation following heart attacks or other heart conditions, and renal rehabilitation for people with chronic kidney diseases. We also offer home health aides for personal assistance with daily activities.
What types of conditions do you deal with?
Rehabilitation therapy is used to promote healing for many health conditions, such as relearning to swallow after a stroke, regaining full mobility and range of motion after a traumatic leg fracture, recovering from a severe heart attack, healing a chronic wound for a bedbound person or shaping a new, physical life for a person who has become paraplegic. Some of the conditions we work with regularly include post-surgical rehab for hip and knee replacement, diabetes management, wound care, heart conditions, COPD, pulmonary fibrosis, traumatic brain injuries, digestive conditions and ostomy care.
Who qualifies for at home rehabilitation?
At home rehabilitation is for people who are recovering from an injury, illness or chronic condition who are unable to go to a facility for regular therapy. At home rehabilitation is not just for the bed bound, but for those who may be somewhat mobile but are essentially home bound.
What’s the difference between home care, home health care and at home rehabilitation?
Home care refers to personal assistance with activities of daily living for those unable to manage these activities for themselves. It includes activities such as support for bathing, grooming, toileting, meal preparation, grocery shopping, transportation, and companionship. Home health care provides professional medical assistance to patients in their homes, usually by skilled nurses and other therapeutic professionals. Home rehabilitation is a type of medical assistance provided in the home that emphasizes healing and recovery from illness or an injury or making improvements for those with chronic and progressive conditions.
Will you work with my physician?
Coordination of care across each patient’s care team is a priority for Pure Health at Home. We communicate regularly with your primary care physician, specialty physicians (such as surgeons, neurologists, cardiologists), all your Pure Health at Home team members (including skilled nurses, therapists and home health aides), family caregivers (including spouses, partners, adult children, close friends) and community services (such as meal delivery, transportation and medical equipment rental). You physician will receive our care plan and regular updates on your progress.
Can my family be involved?
Absolutely! We encourage family caregivers to participate in your healing process as much as you desire and they are available. They can do everything from monitoring your care and providing home care services (such as meal preparation) to hands-on participation with your recovery, such as conducting stretching exercises or massage between our physical therapy visits or learning to give medication injections. We also communicate with your designated family member as often as you like to keep them up-to-date on your care.
Will my health insurance pay for these services?
In most cases, health insurance will pay for our services. Pure Health At Home is reimbursed by Medicare, Medicaid and most private insurance companies regardless of whether the insurance company is in or out of the provider’s network. Our services usually require prior authorization by Pure Health At Home to qualify for coverage.
Do you offer services in my community?
Pure Health at Home primarily works in Ohio’s Cuyahoga, Madison and Union counties.
